I am looking for a simple order form for customers to order additional items (disk space, extra bandwidth, ssl certs etc.). I am hoping there is an already existing solution out there, so I don't have to get it custom built. Here are my requirements:

-hook into Authorizenet

-simple math calculations, so someone can order say 100 extra mbs of space and 5 extra GBS of data transfer and it will be added up correctly

-email orders to specified address

-customizable

These would be nice, but not essential:

-email receipt to customer

-pull CC info from Alabanza database

That's really it. Pretty simple I think. Miva used to have something called "Miva Order" but I guess they discontinued it. Anything out there?
