I am in desperate need of some help in creating an order form.
Here are the details:
I will be offering 4 plans but may change to more in the future.
The form must be customizable where I can change plan info.

Plan1 - Yearly Pay Only
Plan2 - Semi and Yearly Pay
Plan3 - Monthly / Semi / Yearly
Plan4 - Monthly / Semi / Yearly

1- Choose hosting plan
2- Choose payment term
3- enter domain name
4- enter email address
5- payment method (paypal and 2checkout)
7- proceed to choosen payment method

The form can be either perl or php (prefer php)

Anyway, we want to know if you want to store the form values in a database. If yes, we can do that for you.

I am assuming that you have already a set of URLs to direct clients for paying using either paypal or 2checkout.
