I think 2CO's fees are very reasonable.

There are some sites with lower setup fees, but they charge a higher per-transaction fee.

Take a look at www.merchantboss.com, they don't even charge a setup fee, but they charge you a LOT on every purchase.

It's better to invest a few bucks upfront than to keep paying high monthly/transaction fees.

If you are not in the US, 2CO seems the best option. You can check if Paypal accepts your country as well.

If you are in the US, you can contact Corey Bryant or CDGCommerce. They are working in merchant accounts field, and can give you advices

The original poster asked for something with lower setup fees, getting a merchant account through CDGCommerce would have a higher setup than signing up at 2CO.

meanwhile I dont know about CDGCommerce normal setup fee, I believe that CDGCommerce and Corey Bryant at Merchantaccounts4less have special discounts for WHT members. I just want to give customer more options, so, if he's interested, he can contact them :)
