Hi,
I need someone to convert a paper questionnaire into a web form with a php send script and "thank you" page.
More Info:
• There are about 45 Questions/fields.
•I want the email address field validated (i.e. make sure the email address is properly formatted before sending). A few other fields will be "required", and the form must properly handle blank required fields, etc. so the user can re-enter the info.
• I’ll provide you with the 1 page pdf file of the questions and you need to be able to ascertain which are dropdowns, checkboxes, text fields, text boxes, etc.
