This question has probably been asked thousands of times, but on doing a search i cant really find any info thats useful.

Basically im looking for a hosting order form that can be integrated with 2 checkout - if anybody knows of one id be grateful.

You can use http://www.accountlab.com it is free.

The free version is driven by advertising though. If you have the budget I would recommend paying for it, or looking at package like DRAMS, Modernill, or ClientExec.

Does it need to interface with your server? If you don't mind manual setup, you could just use form mail. The first page is the form and collects all the info you need from the customer, then when they submit it, the landing page is the 2CO payment page. You can specifiy either a 1-time 2CO payment or direct it to a 2CO recurring subscription signup page.
