Well, just as I was writing this, I figured out the solution, so I figured I'd share.
Either the OS2 or the PHP 4.1.2 update locks up /home/spool/mail and /var/spool/mail, basically sets them back to the default permissions state. So if you unlocked them to speed up Squirrelmail, unlock both folders and speeds will be back to normal.
