I have been facing a little strange issue with Directadmin, once in a while when you create an account, it tends to hang, the account won't show but you can't create again. Need to ssh in and manually delete the entries in the server.

The server is nearly empty with 0 load, what could be the issue?

What os is it? could be related to quotas.
