I know this has been brought up in the past, but I am curious to know the means by which many of you hosters out there came up with these so-important documents.

Comments are also much appreciated.

I read several of the ones out there.
I found one I liked that I could live with and asked the company if I could use it.
With their approval; I then modified, added, deleted, and basically fudged it around some until it was the way I liked it.

I didn't vote because I didn't see where to include;

*Borrowed from another company (with their permission of course) and added to (among other modifications).

:laugh:... I didn't vote (As my site isn't even finished yet).. but I found it cute that the highest rated answer so far is "Stole it from someplace..."
