View Full Version : Looking for someone to create a form for a website

Hi Everyone!

I'm looking for someone who can create a form that looks something similar to this (http://www.mainlineservices.com/form.jpg). There might be a bit of changes needed to be made, but that's the general idea.

It is for a commercial carpet cleaning and maintenance company. What they want is for a customer to be able to request service online by filling out the form, and have a copy sent to our account manager via email, plus a copy sent back to the client's email address.


It doesn't have to be anything fancy, just looking for something that can be integrated into our template (http://www.mainlineservices.com/template.html) and for something that is simple and works.

The tricky part is that I think our email server is hosted in house, and I'm not a very "tech knowledgable" person.

I can do this job very quickly for $15-$25, depending on how many fields you need, what kind of fields you need, and the mail server situation.

I had to meet with my general manager to get a better idea of what kind of form we need.
