You may want to add CMM level to question 8 as this is the most important factor (at least for us) when choosing a company for outsourced development.

I know this post is really old, sorry to bring it back up.
I have just gotten home from the bars the wife is doing laundry on the bed so i came to the computer and for some god awfull reason remembered this post AND I NEVER GOT MY BLOCKBUSTER CARD. So I called my friend who did this also AND HE NEVER GOT HIS EITHER.
