If you search for "login scripts" on http://hotscripts.com you'll find loads, oxyscripts have one that is aparantley very easy to setup and use at http://phpuserlogin.oxyscripts.com/ , other than that depending on how comfortable you are with PHP there's loads of tutorials around that could help you make your own.
