I am looking for a host that will charge $4/mo or less for the following features

30 MD Disk space
1 GB Data transfer
An FTP account to add/remove files
CGI and PHP capabilities
Would need to install my own copy of Count.cgi

You should try asking this question in the Web Hosting Requests forum (if one of the mods doesn't move this for you) so that hosts can respond with offers.

I host with Affordablehost (http://affordablehost.com) . They have a plan for $45/year that should meet your needs. They only do annual billing on that plan.
