If you are starting out and need something simple (and free) try this: http://www.softopia.com/www.html

It's very simple, takes user input, generates a unique ticket number and emails the customer a receipt and also emails you. Takes about 10 min to setup, and it is very easy to customize or just make it super plain and just call it from an SSI. This the one I'll use for a reseller site I am starting.
