I need a PHP Programmer, that can build me a support panel for my web hosting site.

Pay: $10

What it should be like?
Would like a member login, and when they login, there should be 5 text fields, Name, WebSite, Phone Number, Problem, and etc.
And when they press "Send" i would like that email to be sent to my support email.
