I think Site5 has a plan that would suit your needs.

I think Hosting Matters had some issues with Site5 when it was a hosting clients on Site5 servers.

Well, Site 5 look good - but I ain't too sure about the setup fee thingy..... Plus there doesn't seem to be any activity on their support messageboards
