I need some help regarding forms.
Is it possible to create an online form such that when the user click send, a Word Document be generated (containing the results of the form), the document is attached to the email sent to the specified recipient as well as the person who submits the form? Is there such script available?

I don't know of any such scripts, but this is definitely possible. In fact, so possible, I'm sure it's been done more than a few times and there's probably such a script out there -- just that it may or may not be open source or available to the public. Nonetheless, it very well might be and it's certainly a viable option. Sorry I can't be of any more help than that right now.

I have seen this done to create PDF documents like this, but not Word. Sounds do-able, though.
