Can anyone out there recommend a form processing script that offers the following:

-Supports multi-page forms
-Performs calculations
-Offers email confirmation

I've found a couple that come close, but the calculation is the thing that is missing from most. I need to calculate if the customer needs hosting plan + domain registration + optional services to a total price and display on screen and send to my CC processing gateway.
