I need a online form that asks for the following:
Name:
E-Mail:
Full Mailing address:
Phone:
Car Make / Model:
Race Class:
Comments:

Instead of having this info sent to me via e-mail I would like it placed in a nice file on my server that I can go an look at when I choose.

HTML is fine, PHP would be awesome. This is for a non-profit site, but I will pay a small amount if needed (I do not think it would take a pro long to finish)

this is I can do in an hour or two.. but I'd need access to your server and a database on the server... email me if interested and we'll work out the specifics... doing so in php would not be hard...

Our design guide forms are submission forms that writes into a file. It has an admin script that an admin can view the form submissions online. And he can delete or mark completed.

We will be able to do this kind of job. It is approaching the Holidays. We will be able to start on Monday. Since we don't have much portfolio on this yet, we will be able to make an arrangment of 50% downpayment and 50% upon completion.
