Any windows hosts should support all of your requirements.
Some host providers may charge SQL database addon but usually included free.

I would like to note that some Windows hosts still run Windows server 2003, and even 2000.

I would say there are a good number of hosts out there that should support all that. My recommendation is discountasp.net (http://www.discountasp.net). I've used them for a while and have a few sites there. Always been reliable and I always know that they will have support for the latest technologies, so I don't have to worry if they are compatible for something yet or not.

RC2? do you me R2?

ASP.net 4 is generally available on most windows hosting now. ASP.net MVC 3 is new so you'll need to check with the host but i am sure those hosts that are serious about their hosting services will have this available to you already.

Do you have a budget? what diskspace/bandwidth do you need? how many domains?

they have the features you need except windows server 2008 with RC2

I personally would recommend NetDepot for Windows hosting with SQL Server. Been using them for about 3 years now for high traffic sites and no complaints.

A Windows Cloud Server would work for you. You could install all your required software without issue.

Any shared hosting provider in the industry with featured packed will serve the purpose. Windows hosting packages in almost every host supports asp.net 4 and windows sql server 2008. Look for the reliable and experienced host provider in the industry.

basically all hosting providers provide Windows as well Linux.
asp.net and mssql comes by default with Windows hosting

but yes, some hosting companies charge extra for mssql :)

winhost and web.com provide such features.

cartika hosting provide this for sure on its shared servers.

Decided to go with cloud hosting.

normally shared hosting windows can support. But, you have to check whether the hosting provider have the server. otherwise, can consider a VPS. install the components at your end.
